  Changes to Request for Scheduled Vehicle(s) and/or Driver(s)

          To change a previously scheduled vehicle or driver please e-mail Joyce  Mullins with the old date and the new date for which you wish to reschedule the vehicle.

Cancellation for Scheduled Vehicle(s) and/or Driver(s):
To cancel a previously scheduled vehicle or driver please e-mail Joyce  Mullins with the date for which the vehicle was scheduled.

    Reimbursement Request Form

    If an individual has to pay for gas or service to a State Fleet Vehicle rather than being able to us the assigned Voyager cards, s/he should mail the receipt(s) to PUBLIC SAFETY, along with the completed Reimbursement Request Form (above).
